Insurance and Fees
Dr. Coons is an approved mental health provider with several health insurance plans including Medicare, BCBS (e.g., Anthem, Highmark, etc.) as well as Cigna. She also has a sliding fee scale and provides statements to patients who have out of network benefits. If you are scheduled to be seen for psychotherapy, you will be asked to call C and L Billing Management at 303-280-6262 to provide your contact information and health insurance information. They will confirm your mental health benefits and clarify any co-insurance and/or co-pay related to psychotherapy services.
New patient forms
All new patients who are scheduled for individual or couples therapy or a psychoeducational consult related to fertility care will need to complete the forms listed below. Most of the forms are required by Federal and/or State legislation and/or are consistent with best practices in mental health care to ensure that you are well-informed about psychological services, your rights, and can ask any questions about consultation and treatment services, privacy of information, policies, and procedures. Dr. Coons will forward these forms to you prior to your first appointment or share the link to the forms at www.Carepaths.com when they are available through this platform. Questions about any of these forms and their content are always welcome.
